Project Open Hand’s Homebound/Critically Ill (HCI) program is designed to provide home-delivered nightly meals to patients who are homebound due to a critical medical condition, and otherwise unable to prepare meals for themselves.
Eligibility: The HCI Program serves eligible San Francisco and Alameda county residents for six months based on the following criteria:
- Patient is recovering from a recent (within the past 30 days) trauma, injury, or surgery that renders them homebound and otherwise unable to prepare a meal for him or herself, OR
- Patient is experiencing an acute exacerbation of a chronic condition, which temporarily renders them homebound, OR
- Patient has a terminal diagnosis
